What is acid reflux?

Acid reflux is a problem that affects most people over the course of their lifetimes.  It can persist for many years before a definitive solution is found.  Many sufferers try antacids and other acid-reducing medications with variable success.  Acid reflux can worsen during periods of stress or weight gain making it particularly difficult to address long term.  If severe GERD continues to persist, you should speak to Dr. Dasari about anti-reflux surgery.

Anti-reflux surgery is often considered for people who have the following:

  1. Significant symptoms of heartburn, esophageal metaplasia (Barrett’s), or chronic sinusitis
  2. Difficulty eating certain foods
  3. Hiatal Hernia
  4. Problems with Proton-Pump-Inhibitor or H2-blocking Medications

Anti-reflux surgeries, which are widely performed, come in four types:

  1. Nissen Fundoplication (with or without concurrent hiatal hernia repair)-  This procedure is performed with small incisions (5mm- 1 cm), using minimally invasive techniques (either laparoscopic or robotic), with generally very good results. It involves wrapping the upper portion of the stomach around the esophagus to re-create the valve effect from the lower esophageal sphincter.  Generally the procedure is well tolerated with a short recovery.
  2. Linx procedure- The Linx device is an implantable metal magnet that forms a ring around the bottom of the esophagus.  These can be installed laparoscopically or robotically.  Generally the risks associated with the implant are low, but include device malfunction, wear and tear, migration and erosion.
  3. Transesophageal, Incisionless Fundoplication- TIF is performed with an endoscope placed through the throat, and utilizes a plication device to re-create the lower esophageal fold with the stomach.  If you have a concurrent hiatal hernia that is large (>2cm), this can be repaired surgically during the same setting.
  4. Gastric Bypass (Roux-en-y) Surgery- If reflux is associated with a body mass index in excess of 35, then a specialized procedure to address both obesity and acid reflux may be considered.  However, if weight loss can be achieved in a reasonable time frame, clients can avoid major surgery in most cases.

What to expect before anti-reflux surgery

If you would like to avoid permanent implantable devices, Dr. Dasari offers several options for anti-reflux surgical repair for GERD with a reduced risk of complications.

Before surgery, a detailed workup is conducted to ensure that your symptoms will be properly addressed with the correct technique.  Many times, acceptable non-surgical solutions are discovered.   Often times, during the process, special diagnostic tests can help determine the particular causes of gastroesophageal reflux symptoms.
